Job description:

Our agency serves Aiken County, Barnwell County, Greenwood County, Orangeburg County, Saluda County, McCormick County, Edgefield County, Lexington County, Allendale County, and Bamberg County.

case manager needed full-time to identify needs and implement the provision of services within the community for individuals who have Autism, Intellectual Disabilities, Head and Spinal Cord Injuries or other disabilities and special needs. Job requires significant travel within the community, home visits, assessing needs, developing plans, monitoring services, and working with providers.

Qualifications: Bachelor’s degree or higher and have at least one (1) year of experience working with people with disabilities or one (1) year of Case Management experience.

OR

Registered Nurse licensed in the State of South Carolina and have at least one (1) year of experience working with people with disabilities or one (1) year of Case Management experience.

All degrees must be from a college or higher education institution recognized by the US Department of Education/the Council for Higher Education Accreditation (CHEA).
