Case management services provides individuals with intellectual disabilities, related disabilities, autism, head injuries, spinal cord injuries and similar disabilities with advocacy, support, and monitorship of services. Our services are provided by trained professionals with knowledge of local resources within the community. There are 3 types of case management services provided by the agency.
State Funded Case Management
State funded case management services are provided to individuals who do not have Medicaid or individuals who have Medicaid but are enrolled in a waiver that is not managed by a DDSN provider. Individuals who receive state funded case management services may be eligible for respite services or family support funds, if no other resources are available. Medicaid Targeted Case Management
Medicaid targeted case management services are provided to individuals who have Medicaid but are not enrolled in a waiver. If you have on going needs, MTCM will be requested for you by your case manager. Your case manager will complete an assessment and develop a plan to address your needs. Individuals who receive MTCM may be eligible to receive respite services and family support funds. If you have immediate needs, please contact our office for assistance.
Waiver Case Management
Waiver case management services are provided to individuals who have been awarded a waiver slot through DDSN. Waiver slots provide additional services which are not funded by the State Medicaid Health Plan. There are 3 waivers which are administered by DDSN providers: Intellectual Disabilities/Related Disabilities Waiver (ID/RD Waiver), Community Supports Waiver (CSW), and the Head and Spinal Cord Injury Waiver (HASCI Waiver).
Currently, there is a waiting list for the ID/RD Waiver and the Community Supports Waiver. WCM services include monthly contacts by your case manager, quarterly face to face visits, and annual assessment and planning. Your case manager will offer you a choice of qualified providers for all of your service needs. Your case manager will also monitor the usefulness and effectiveness of each service you receive monthly.
